ACNE Banish Blemishes and Invite Income Zits---pimples---blemishes---no matter what we call them, acne can be distressing and annoyingly persistent. Lesions often heal slowly. When one begins to resolve, others seem to crop up. This ongoing battle and long-lasting cycle is both wearisome and frustrating to your patients. Acne is the term used for plugged pores (blackheads and whiteheads), pimples, and even deeper lumps (cysts or nodules). Acne occurs on the face, neck, chest, back, shoulders and even the upper arms. Hormones play a role in the development of acne so it affects most teenagers to some extent. However, the disease is not restricted to any age group. Even adults into their 40's can get acne. While it's not a life threatening condition, acne can be upsetting because of its unsightliness. Some cases of acne even lead to serious and permanent scarring. All acne is a disorder of the pilosebaceous unit, which is made up of a hair follicle, a sebaceous gland, and a hair. Sebaceous glands produce a substance called sebum, which is responsible for keeping skin and hair moisturized. During adolescence, sebaceous glands enlarge and produce more sebum under the influence of hormones. Bacteria, known as Propionibacterium acnes that is a normal inhabitant of the skin, and uses sebum as a nutrient for growth, therefore increases in follicles during puberty. People with acne have more Propionibacterium acnes in their follicles than those without acne. The presence of these bacteria attracts white blood cells to the follicle. These white blood cells produce an enzyme that damages the wall of the follicle, allowing the contents of the follicle to enter the dermis. The bacteria also cause the formation of free fatty acids, which are irritants, increasing the inflammatory process in the follicle. Problems arise when the inflammation causes sebum to get trapped in the hair follicle and some hair follicles become obstructed. The cells that are normally sloughed off become sticky, plugging up the follicle. It causes some follicles to form comedones, also known as blackheads and whiteheads. Normally when the follicle is "full", the sebum spreads over the skin surface. When this process works correctly, the skin will be moisturized and remain healthy. When it does not, acne occurs. Acne can take months or years to treat successfully. Depending on its severity, acne can cause emotional distress and lead to scarring.
